New: Supporting Wellbeing Projects

Older people who are frail or have complex medical needs have a very high risk of being socially isolated. Health and mobility problems mean that getting out to do the things you enjoy becomes increasingly difficult, and extra support can help.

The Supporting Wellbeing projects connect with professionals such as GPs, memory support teams, community matrons and more, to reach people who are in need of that extra level of support. 

All projects will take a person-centred approach, seeing an individual as just that–an individual, not a collection of illnesses or impairments. The process starts with a home visit to get to know the person. They will work with the individual to find out their goals and a reasonable path toward those goals. 

The service will not be time-limited, as it is understood that people who are frail may require long-term engagement to be able to participate confidently in activities.
