As Enhance enters its fourth year of delivery, the programme is consistently proving how sustained partnership working, flexible delivery, and community-rooted approaches can meaningfully reduce clinical demand while improving patient wellbeing and independence.
The British Geriatrics Society have published an article in their member magazine, AGENDA, which shares the latest evaluation findings, highlights from frontline delivery, and reflections on how the model is evolving to meet emerging needs. As we look to the future, it also offers a timely call to action: to strengthen cross-sector relationships and secure long-term investment in what is proving to be a vital component of integrated care for older people.
Delivered by Leeds Older People’s Forum (LOPF) in partnership with Leeds Community Health Care Trust and four third sector delivery partners based across Leeds, Enhance continues to make a positive difference to the care and wellbeing of older people.
